Community-Based Residential Options
Independent Apartments
Cedar Lake independent apartments offers supported independent living options for individuals who need less intensive support and wish to live independently in the community. Participants are encouraged to engage in community employment, day programs, or other community activities.
Family Home Provider
Family Home Providers (FHP) contracted through Cedar Lake offer ongoing care for individuals with Intellectual and Developmental Disabilities (IDD). Participants live in the same household as the FHP, enjoying a private bedroom and access to communal living spaces. This service allows individuals to live within the community and engage in everyday life experiences such as shopping, dining out, and vacations.
Group Home
The Cedar Lake Group Homes are typically for individuals with mild or moderate disabilities, this option provides a semi-independent, 24-hour staffed home for six residents. Those living in the group home receive training and support in daily living skills, transportation, leisure and recreational activities, and benefits coordination.
Staffed Residences
Cedar Lake Staff Residences offer 24-hour care for three individuals needing more intensive support in a single-family home setting. Residents in our staffed homes enjoy various opportunities and choices for employment in competitive and small business settings, day programs, and cultural and recreational activities.
Intermediate Care Facilities
Cedar Lake Lodge, Park Place and Sycamore Run are the three licensed Intermediate Care Facilities that Cedar Lake offers. These locations all have 24-hour nursing care, occupational, speech and physical therapy in addition to psychological and behavioral services.
