A Family Home Provider Story

Nestled in the heart of LaGrange, Bonnie and Jason Jacobson’s home is more than just a house—it’s a place where love, care, and connection thrive. As a Family Home Provider (FHP) with Cedar Lake, Bonnie has created a nurturing environment where individuals with intellectual and developmental disabilities can flourish. Alongside her husband Jason, who serves as a co-FHP, Bonnie has made it her mission to provide a safe, loving home for those in need.

Bonnie has been with Cedar Lake since 2015, beginning her journey as a direct support professional in Cedar Lake Residences. Her passion for helping others led her to become the first individual to join Cedar Lake’s Family Home Provider program. Bonnie’s dedication to the individuals she supports is evident in her words: “I love people, and I like making their lives better.”

The First Family Home Provider at Cedar Lake

The Jacobsons currently support two individuals, Billy and Melanie, who have become integral members of their family. Billy, the first person supported through Cedar Lake’s FHP program, joined the Jacobsons in March 2021, while Melanie became part of their household in March 2024. Bonnie recalls how Billy’s mother was initially apprehensive about the change in living arrangements. However, after meeting Bonnie and her family, seeing the home, and doing trial days with Billy, his mother quickly realized this was the best decision for her son. Today, Billy is thriving, having made significant improvements in many areas of his life. Bonnie says, “It feels like he has always been here.”

Melanie, who had previously lived independently in a Cedar Lake apartment, needed more assistance due to medical conditions. The Family Home Provider program offered her the perfect solution, allowing her to be around people who could provide the necessary support while still maintaining a sense of independence. Melanie settled into the Jacobson home seamlessly, already familiar with Billy and AJ, Bonnie’s son, through their shared time at the Cedar Lake Enrichment Center (CLEC). Melanie, who loves to cook, now enjoys preparing dinner once a week with Bonnie, choosing recipes and shopping for ingredients together.

The Jacobsons’ home is a lively place filled with activities and adventures. They attend church every Sunday, have fun bowling, playing putt-putt golf, and swimming in their pool. Melanie loves the water and often plays water basketball, while Billy enjoys gardening, playing with the family dogs, and keeping an eye on everyone in the house. Together, they have become a close-knit family unit that looks out for one another.

One of the many joys the Jacobsons share with Billy and Melanie is planning and going on trips. They are currently organizing a vacation to Cincinnati to visit the zoo, watch a Reds baseball game, and explore the aquarium. These outings are not just about having fun; they are about creating memories that last a lifetime.

A Rewarding Job

Bonnie describes her role as an FHP as the most rewarding opportunity she could ever have. “I can’t imagine our family without Billy and Melanie. We just mesh so good,” she says. Bonnie believes that being an FHP is like adding another family member, providing them with opportunities and acceptance, something that can be challenging for individuals with intellectual and developmental disabilities. She is proud to be their voice, advocate for their needs, and ensure they feel valued and loved.

For anyone considering becoming a Family Home Provider, Bonnie offers heartfelt advice: “It is so rewarding, I can’t imagine not doing it. It feels like I have always done this. Just to know that you are able to give someone a life they may not have had. They make me so happy to see them excel, like how awesome Melanie is doing now. I am so proud of her.”
