Community-Based Services

Cedar Lake Residences supports 115 individuals throughout Oldham, Jefferson, Henry and Shelby Counties through a variety of person centered, holistic services. Our goal is to engage individuals in their community while promoting independence, self-sufficiency and spiritual health.

Community Living Support

Community Living Supports are provided by a Direct Support Professional (DSP) and emphasize goals and objectives related to community integration. Supports may be provided in the individual’s home or community setting.

  • Routine household tasks and maintenance

  • Shopping
  • Money management
  • Medication management
  • Socialization
  • Relationship building
  • Leisure choices
  • Participation in community activities
  • Therapeutic goals
  • Nonmedical care not requiring nurse or physician intervention

Homemaker Services

Homemaker Services Supports are provided by a Direct Support Professional (DSP) with a focus on domestic activities and pertinent life skills that occur in the individual’s home. The goals and objectives are person-centered with the purpose of gaining or maintaining independence.

Community Access

Community Access provides support and guided experiences to encourage an individual to develop a network and connection with natural relationships in their community. This service is provided by a certified Community Access Professional and entails collaboration with the individual to independently become a part of their community. The individual’s goals are specific, person-centered and include defined benchmarks with fadeout criteria. This empowers the individual to engage in meaningful events and build relationships without professional support.

Personal Assistance

Personal Assistance services enables an individual to accomplish person-centered goals and objectives with the support of a Direct Support Professional (DSP). The intention of this support is to help the individual acquire independent skills that can be provided in the home or community.

Office Of

Vocational Rehabilitation

Vocational rehabilitation services assist an individual with their goals to reach their employment outcome. Counselors are assigned to each case to discuss guidance and assessments that focus on person-centered initiatives. Contacting a vocational rehabilitation counselor is generally the first step in order to work toward long-term supportive employment supports.

Supported Employment

This service is provided by a certified Employment Specialist whose primary focus is to obtain and maintain a meaningful job to an individual in the community. This includes searching for competitive employment to earn a living wage and a satisfying career while building natural relationships and in the workplace. The Employment Specialist offers a person-centered approach with flexibility while ensuring reasonable accommodations are in place for growth and advancement opportunities.

Adult Day Training Programming

Cedar Lake Enrichment Center (CLEC) offers onsite and community-based programming for individuals with intellectual and developmental disabilities. The enrichment center offers a variety of activities that assist with socialization to build and maintain connections and friendships. The enrichment center strives to incorporate a variety of experiences with vocational training and programming specific to physical and emotional wellness. Our mission is to provide group activities that enrich the individual’s life through meaningful interactions.
