Family Home Provider Program

Cedar Lake’s Family Home Provider (FHP) program connects adults with intellectual or developmental disabilities (IDD) to caring, supportive homes in the community. Whether you are a family exploring options for your loved one or someone interested in becoming a provider, the FHP program offers a personalized, family-style approach to care.

For Participants and Families

What it is:

  • Live in a private bedroom within a supportive, family-style home
  • Access shared living spaces and participate in daily routines
  • Enjoy community experiences: shopping, dining, events, and vacations
  • Receive services under the Supports for Community Living (SCL) waiver, including Community Access, Supported Employment, and Day Training

For Prospective Providers

  • Contracted through Cedar Lake to provide ongoing care in your home
  • Care for an individual(s) with IDD in a supportive family environment
  • Short-term Co-FHP roles available to provide relief and support for other providers
