Family Home Provider Program
Cedar Lake’s Family Home Provider (FHP) program connects adults with intellectual or developmental disabilities (IDD) to caring, supportive homes in the community. Whether you are a family exploring options for your loved one or someone interested in becoming a provider, the FHP program offers a personalized, family-style approach to care.
For Participants and Families
What it is:
- Live in a private bedroom within a supportive, family-style home
- Access shared living spaces and participate in daily routines
- Enjoy community experiences: shopping, dining, events, and vacations
- Receive services under the Supports for Community Living (SCL) waiver, including Community Access, Supported Employment, and Day Training

For Prospective Providers
What it is:
- Contracted through Cedar Lake to provide ongoing care in your home
- Care for an individual(s) with IDD in a supportive family environment
- Short-term Co-FHP roles available to provide relief and support for other providers
