FAMILY HOME PROVIDER SUPPORT
At Cedar Lake, your loved one is not just a participant — they are a valued member of a caring family. Our Family Home Provider (FHP) program offers a warm, supportive environment where individuals with IDD experience personalized care, community engagement, and a true sense of belonging.
Life as a Participant
Living with a Family Home Provider means enjoying personalized support tailored to your loved one’s routines, preferences, and goals. Residents are matched carefully with providers to ensure a strong, meaningful connection, creating a home where care and relationships flourish.
Participants enjoy:
- Private bedrooms with access to shared living spaces
- Community experiences such as shopping, dining, events, and vacations
- Comprehensive services under the Supports for Community Living (SCL) waiver, including Community Access, Supported Employment, and Day Training
- Opportunities for long-term stability with the option to transition to Cedar Lake’s Intermediate Care Facilities if additional medical or specialized support is needed
