As a contracted FHP, you provide a safe, nurturing home environment while supporting independence, growth, and community participation.

 

What is a Family Home Provider (FHP)?

Family Home Providers are individuals contracted through Cedar Lake to provide ongoing care for individuals with IDD. Residents live in the same household as their FHP, enjoying a private bedroom and access to communal living spaces. This arrangement allows participants to experience daily life in the community, including shopping, dining out, attending events, and vacations.

Co-Family Home Provider (Co-FHP)

Co-FHPs provide short-term relief and support for individuals in primary FHP households. These supports help cover vacations, emergencies, or other temporary needs. Co-FHPs complete the same training as primary FHPs and are managed through a contract with Cedar Lake.
